Opening in early 2027, the Hartland Frail Care Centre has been designed from the ground up with one principle in mind: that every resident deserves to feel truly at home, regardless of their care needs.
When recovery requires more than rest, Hartland’s Sub-Acute Facility delivers. Operated by CareVita, this is a space where clinical excellence meets genuine compassion. Whether you’re recovering from surgery, managing a complex condition, or transitioning back to independent living, every resource you need is right here — within the community you already call home.
CareVita provides clients with personal panic buttons through our partnership with Telecare. These devices offer immediate access to emergency support at the press of a button. We assist with the installation, setup, and training to ensure every resident knows exactly how to use their device. When a panic button is activated, our team is notified and responds without delay. For medical emergencies, we coordinate directly with ER24, our trusted ambulance and emergency medical response partner.
This system ensures that help is always close at hand – 24 hours a day, 7 days a week.
Our doctor offers accessible and personalised medical care through phone consultations, complemented by home visits whenever needed. This approach ensures that every client receives timely guidance, thorough assessment, and ongoing management of their health in a way that is convenient and comfortable. Home visits are coordinated closely with our nursing team to provide a seamless, integrated approach to care, addressing both routine and urgent medical needs while supporting overall well-being.
At CareVita, we take an integrated approach to care, one that goes beyond physical health. We recognize the importance of emotional and social well-being in maintaining a good quality of life, especially for the elderly and vulnerable. That is why we regularly host social gatherings, group activities, and community events to encourage connection, joy, and a sense of belonging. Through these engagements, we aim to reduce isolation, uplift spirits, and build meaningful relationships among our clients.
Comprehensive health assessments conducted to meet medical aid requirements. These checks include measuring vital signs, reviewing medical history, and assessing overall health status to support claims or compliance with medical aid protocols. Assessments are conducted by the registered nurse and, where necessary, coordinate with the attending doctor to ensure accurate documentation and recommendations for ongoing care.We also assist with mobility, feeding, hygiene, and continence care, always ensuring comfort and dignity. Our nurses work closely with families.
Doctors and other healthcare providers deliver coordinated, high-quality, practical, day-to-day support they need to feel secure and cared for.
From mid-October, the registered nurse will host scheduled clinic days on-site. These sessions will provide residents with convenient access to professional nursing care, including health assessments, wound care, chronic condition monitoring, medication reviews, and health education. Where needed, referrals will be made to the attending doctor or other healthcare professionals to ensure comprehensive care.
